Enrolment

Juno Health is currently accepting new patients and we warmly invite you to enrol.

To begin the enrolment process, please click on the “Enrol Now” button provided below. You are also welcome to complete the form in person at our clinic.

Enrolment Process:

  • To ensure continuity of care and for us to provide the best possible service, we require new patients to consent to the transfer of their medical records from their previous doctor. No medical notes will result in no enrolment due to the medico legal risks. If you are new to New Zealand and have not previously been enrolled at a medical centre, please contact reception.

  • Please be aware that there is a current waitlist of approximately 2 weeks. Enrolments are processed in the order they are received, subject to our capacity.

  • Once we have entered you into the system and received your medical notes (if applicable), you will be notified that your enrolment is complete via email. Please note it can take up to 14 business days for your enrolment to be completed.

  • If you require medication before your enrolment is complete, please contact your current medical centre to arrange this.

New Patient Consultation:

  • Please note that as you are new to our clinic, anyone over the age of 18 years or on regular medication will need to have a new patient appointment. This appointment is 30 minutes with the doctor and 15 minutes with the nurse and is so we can get a thorough understanding of your health background and update any information needed to your file. This is at a cost of $130 or $80 if you have a community service card. You will be able to book this appointment once your enrolment is complete. Please note that we require pre-payment for all new patient appointments.

Communication and Policies:

  • Juno Health uses text communication for appointment reminders and recalls. By completing your enrolment, you automatically consent to this. If you wish to opt out, please notify us via email.

  • Please review our practice policies here. By enrolling, you agree to these policies.

  • You will be automatically signed up to HealthOne, a secure electronic health record. Information regarding HealthOne is here. If you wish to opt out, please contact HealthOne directly through their website.
